FSST Medical Marijuana Program Forms
Please bring a valid photo ID, completed application & printed physician recommendation to the above address.
* IN PERSON ONLY, mailed or faxed documents/payments will not be accepted. *
FSST will accept valid SD or out of state medical cards in place of physician recommendation.
We accept valid South Dakota or out of state medical cards at no extra charge.
Have a certified physician recommendation? We accept Telehealth or local physicians.
Fill out our application, pay the flat fee of $50.00, your card is printed onsite, making you immediately eligible to purchase our dispensary products.
Please note: Medical records are NOT an acceptable form of proof of need. The on-site staff are not healthcare providers and cannot issue a card based off opinion or records. We encourage anyone seeking a card to speak with their primary care provider or utilize one of many Telehealth options available.
Per FSST Tribal Ordinance, Section 29-2-3(b)’s language does expressly prohibit individuals under 21 years of age on the premises. Therefore, a designated caretaker would be needed to obtain a medical card for an individual under the age of 21.
