This application is for both new and returning patients. Please bring a valid photo ID & printed physician recommendation (when applicable) to the above address.
* IN PERSON ONLY, mailed or faxed documents/payments will not be accepted. *
Please note: Medical records are NOT an acceptable form of proof of need. Our on-site staff are not healthcare providers and cannot issue a card based off opinion or records. We encourage anyone seeking a card to speak with their primary care provider or utilize one of many Telehealth options available.
